Getting older doesn’t mean you have to stop calling the shots when it comes to how you live your life. We understand that your independence matters to you. Uniting can help you stay in your own home, with the level of support that suits you.

We take the time to sit down with you to learn about your preferences, priorities and needs, and your goals for your health, wellbeing and lifestyle.
Our goal is to partner with you to develop a personalised plan, so that you can continue to live at home happily and safely. Together we will regularly review this support plan to make sure that everything is on track. We know that your goals and needs may change over time, so these regular check-ins will help make sure that you receive the support you want.
A member of the Uniting team can discuss the cost of your personalised home care package with you. Fees will vary from person to person. This is partly due to government funding legislation and the different service options you may choose.
We’ll provide you with all the information you need and work with you to develop a personalised budget.
Here’s what you need to do to access home care services through Uniting.
We’re here to help you to achieve your goals. You can get in touch to share your story and find out more about the support we provide. It’s also okay for one of your friends, a family member, carer or your doctor to contact us on your behalf for more information. Call our home and community care team on 1800 864 846 (Monday to Friday, 8.00am–6.00pm).
To get things started, you will first need an assessment to check your eligibility for home care services. A member of your local Aged Care Assessment Team (ACAT) will talk to you about your current situation and check your eligibility by completing a free assessment. We can put you in touch with your local ACAT, or if you would like us to give you advice before or after your appointment, please do not hesitate to contact us. We’re here to help.
A member of the Uniting team will visit you at home to discuss your needs, preferences and goals during a care conversation. We know that everyone is unique and likes to do things their own way, so it’s up to you how we work together. Whether you’d prefer to manage your services yourself, would like a bit of help handling things or would like us to look after your personalised care package from start to finish, we have a partnership option that will work for you.
