How to access home care services through Uniting
1. Get in touch with us
We’re here to help you at every stage of the process. It can appear a little daunting, so we’re here to make it as easy as possible.
Just call our team on 1800 864 846, Monday to Friday, 8.00am to 6.00pm. Or if you prefer, ask a family member, friend, carer or doctor to call us on your behalf. We’ll explain the whole process in a way that’s easy to understand.
2. Contact My Aged Care
The next step is to contact the Australian Government department that deals with home care, My Aged Care. They’ll discuss your eligibility and arrange an assessment.,
3. Arrange an in-home visit and assessment.
An assessor will visit you at home to talk to you about your needs, and the sort of care and support you might want. This assessment will determine whether you are eligible for a Level 1, 2, 3 or 4 Home Care Package. It’s always good to have a family member, friend or carer at the assessment, for a little extra support. This assessment is free of charge.
After you’ve had your in-home assessment, you’ll receive a letter letting you know what level of Home Care Package you’ve been approved for: Level 1, 2, 3 or 4. You’ll then join a waiting list to be assigned your home care package. Remember, you can opt to accept a lower level of package while you wait for your approved level to become available, just let your assessor know.
4. Book a care conversation with Uniting.
You’ll receive a letter when your Home Care Package is assigned then it’s completely up to you who provides your care, you’ll find a comprehensive list of providers on the Australian Government My Aged Care website.
If you do decide to talk to us, we’ll send a member of our Uniting team – your support worker – to visit you at home for what we call a care conversation. You’ll be able to chat through everything including, what your needs might be, the goals you might have and the type of care you’re thinking of. And we’ll be able to explain all our services and how we’ll organise your care.
Your support worker will then put together your detailed care plan, so you can continue to enjoy living independently in your own home.

On Good Friday (19 April), Easter Monday (22 April) and ANZAC Day (25 April) our Customer Service Centre will be open from 10am to 2pm.
