Aged Care Assessment Team (ACAT)
Have you been advised you require an aged care assessment or Aged Care Client Record (ACCR)?
An ACAT assessment is a requirement for many Commonwealth funded programs that support people to live at home or access residential care and/ or residential respite.
The Waverley ACAT provides a comprehensive assessment for people aged 65 and over (or 50 and over for Aboriginal and Torres Strait Islander seniors) by visiting you in your home. The team is made up of registered nurses, social workers and administration staff.
How we can help
The Waverley Aged Care Assessment Team can:
- Undertake comprehensive assessments
- Complete an Aged Care Client Record (ACCR)
- Recommend Transitional Aged Care Programs (TACP)
- Review the level of care required and make changes as needed.
If you live in one of the below suburbs, a referral for an assessment from the Waverley ACAT can be made by yourself or by a doctor, health professional, relative or friend by ringing the Northern Network Access Referral Centre (NNARC) on (02) 9369 0400.
